MST Therapist
Assessment Responsibilities:
- Ensure that intake forms (including research materials) are properly and accurately completed.
- Ensure that the client has had ample opportunity to provide all information necessary to produce an accurate and complete assessment of family strengths and needs.
- Determine an appropriate preliminary diagnosis and pre-discharge planning to be included in assessment materials.
Behavioral Healthcare Treatment Responsibilities:
- Staff each new case with other therapists and in MST supervision and consultation in a peer review format to share preliminary treatment plans and receive feedback.
- Involve the consumer in the formation of the treatment plan.
- Write the treatment plan according to MST standards, incorporating feedback from the client and the peer review process, and includes overarching goals and intermediate objectives.
- Review and update the treatment plan and documents the review process in accordance with program policy and MST standards
- Coordinate appropriate services to successfully complete treatment plan and coordinate with other programs to facilitate integration of MST within the continuum of services.
- Deliver all services according to MST principles and practices.
Crisis Response Responsibilities:
- Provide crisis response services including taking emergency phone calls, and making emergency visits as needed for one week at a time as part of the MST program on-call rotation.
- Attend to emergencies as they arise whenever on-duty and available, including handling emergency phone calls and making emergency visits as necessary.
Operational and External Relationship Responsibilities:
- Maintain adequate availability for clients and those in their system.
- Accurately and consistently complete and maintain all required documentation and paperwork in a timely manner.
- Complete regular reviews of client charts and treatment plans and document such reviews according to Mid-Plains Center’s policy.
- Develop and foster a working relationship with referral sources, including, but not limited to: School personnel, healthcare professionals, law enforcement and judicial officers, mental health professionals, and social services agencies and personnel.
- Follow up on all referrals received to ensure that clients receive appropriate and timely services.
- Follow up on no-shows and continuously improving quality of services to ensure client retention.

Qualifications
- Education and licensure (either/or provisional or fully licensed) as a Mental Health Practitioner.
- Excellent communication skills (written and spoken). Bilingual fluency in English/Spanish is desired.
- Must have reliable transportation and be willing to transport consumers as needed.
